The Home Based Care service provides support to individuals with health challenges, particularly those affected by chronic illnesses. Services include medical assistance, provision of essential supplies, and social support to improve the quality of life for patients and their families.

1. Case finding, anti-retro viral therapy (ART) abscond and emphasize ART adherence and refer to care and treatment clinic (CTC) and voluntarily counseling and testing centers (VCT).
2. Recognizing people with chronic illness, people with disabilities and identify their needs and social support then support or refer to the appropriate stakeholders.
3. To provide health education to care taker and counseling to patients with chronic illness.
4. To provide social support like medicines, food, mattress, clothes etc.
